Started training scouting again. There may be others with more ranks in the following, though for history sake ... no Force of Nature title with the following ranks:
Circle: 123
Athletics: 988 22% rapt (30/34)
Perception: 904 49% rapt (30/34)
Outdoorsmanship: 1007 25% rapt (30/34)
Scouting: 900 06% riveted (28/34)
I'll post again once all are over 1000. The ranks could have been bumped up though since now there's no swimming.

Entity of Nature also has spell requirements that won't work until magic is off preview.

Corik's Wall and the Branch in Ilithi. Corik's is the primary trainer, I've been training there since about 650-700's, so probably a few years ago.

No, but there was a spell requirement before (caiman swim), so probably requires the Athletics spell now. May show up after the spell preview is over.
