I had a parry stick on, but was set to shield & evasion. Kept getting this message occasionally.

A ghoul rakes its sickly green claws at you!
The ghoul's claw clashes with your , the sound grating harshly on your ears.

I think the ghoul was clashing with your outfit. You need to do better picking out your cloths to fit the critter you are fighting!
